A limited number of scholarships will be available for current dues-paying members of the National Minority AIDS Council (NMAC). Scholarship applicants can select from either:

Option A

Registration only

or

Option B

Registration, $100.00 travel subsidy and two nights lodging at the official conference hotel.

Scholarship recipients who qualify for Option B will receive the travel subsidy on- site when they register for USCA. To receive the two complimentary nights of lodging at the official conference hotel, recipients must make their own hotel arrangements, and submit the room confirmation number to the conference registrar if they are awarded a scholarship.

Though all staff members, directors and volunteers who meet the above criteria are invited to apply, only one scholarship per agency will be awarded.

Scholarships will be awarded based on financial need, geographic distribution and population(s) served. Also considered are the skills the participant will be able to use in his/her agency and community. Applications will be accepted from current dues paying members of NMAC only. Please provide verification of NMAC membership. Scholarship applicants with questions about their membership or constituency status should contact NMAC prior to returning this form.
We encourage you to apply online at www.2015usca.org. Please submit completed scholarship applications on or before June 19, 2015, by 5:00 pm (EST).
Applications sent by fax, or received after the deadline will not be considered.

Scholarship Application

In order to apply for a scholarship to attend USCA, we encourage you to fill out our online application form. Please note: The confidentiality of contact and demographic details provided on USCA forms – including name, mailing address, zip code, telephone number, gender and HIV status – cannot be guaranteed. The registration and scholarship selection processes often require that information collected be shared with NMAC staff, USCA partners and corporate sponsors.

To ensure that people with HIV/AIDS are awarded scholarships, there is an optional section where applicants can provide their HIV status. Please know that the entire contents of scholarship applications– including HIV status – is provided to reviewers. In addition, USCA partners and corporate sponsors may use your contact information to send e-mails and other communications regarding events at USCA. To opt out of receiving such messages, please send an e-mail request to communications@nmac.org.
